Patient centered and family focused care provided by a team of palliative care doctors, nurses, social workers, aids and others working together to manage chronic serious illness to improve quality of life.
Hospice
We provide comfort and care designed to meet the physical, emotional, and spiritual needs of terminally ill clients and their families in the place that they call home. This could either be a house, apartment, hospital room, assisted living/group home, and nursing home, or anywhere else a patient calls home.
Hospice Team
What to expect under our Hospice care:
- We manage patients’ pain and symptoms
- Assists the patient with the emotional and psychosocial and spiritual aspects of dying
- Provides needed drugs, medical supplies, and equipment.
- Coaches the family on how to care for the patient
- Delivers special services like speech and physical therapy when needed
- Make short term inpatient care available when pain or symptoms become too difficult to manage at home, or the caregiver needs respite time.
- Provides bereavement care and counseling to surviving family and friends
